T Cell Subsets in Peripheral Blood and Serum Levels of Soluble Interleukin-2 Receptor Levels in Patients with Chronic Liver Diseases


Abstract
To evaluate peripheral T cell subsets and biological significance of serum soluble interleukin-2 receptor (sIL-2R) levels in patients with chronic liver diseases, the author analyzed T cell subsets, using two-color flow cytometry, and serum
sIL-2R
levels by sandwich enzyme immunoassay in 14 cases of chronic active hepatitis(CAH), 20 cases o liver cirrhosis(LC) and 15 cases of hepatocellular carcinoma(HCC) as well as 19 cases of adult healthy subjects as control.
Diagnoses were confirmed histopathologically in all of the patients. The patients of CAH included 8 cases of CAH type B, 4 cases of CAH type C and CAH with unknown causes. The patients of LC included 14 cases of HBsAg positive LC, 5 cases of
alcoholic
LC and one case of anti-HCV positive LC. All blood samples were obtained from veins of the patients and control subjects in Pusan National University Hospital during the period of January to August, 1993.
@ES The results obtained were summarized as follows:
1. The percentage of T cell subsets was not different between CAH(type B and type C) and adult healthy subjects.
2. The percentage of CD8+ and Leu2a+ Leu15+ cells were significantly decreased in LC(HBsAg positive and alcoholic),and CD4/Cd8 ratio was significantly increased in LC(HBsAg positive and alcoholic) as compared with adult healthy subjects.
3. The percentage of CD4+ and Leu3a+ Leu8+ cells, and Cd4/Cd8 ration was significantly increased, and CD8+ and Leu2a+ Leu15+ cells was significantly decreased in HCC as compaired with adult healthy subjects.
4. The serum levels of sIL-2R were significantly higher in CAH(type B and C), LC(HBsAg positive and alcoholic), and HCC as compared with adult healthy subjects.
On the basis of this results, it is suggested that the Immunodysfuction in liver cirrhosis patents is partially due to decreased cytotoxic T cells of peipheral blood lymphocytes, while immunodysfuction in hepatocellular carcinoma is partially due
to
increased suppressor inducer T cells and decreased suppressor T cells of peripheral blood lymphocytes. It also suggested that sIL-2R may be a marker providing status of cellular immune response in patients with chronic liver diseases.
